In 2007, journalists for Reuters, Namir Noor-Eldeen and Saeed Chmagh were killed by US forces. Reuters kept pressing for information about their deaths. The US government stonewalled, of course.
Here's a tribute to Namir with some of his war photos. It's too apologetic to the US military in my view. The first photo isn't gory, but it does show an RPG, which will be useful information if you follow the next link.
Here's the actual video of the Apache helicopter firing on the journalists, leaked by Wikileaks. Note the language they use to rationalize their actions, especially after the fact.
